What symptoms should prompt a menopause consultation?

Menopause is far more than hot flushes. That misconception leads many women to dismiss symptoms that are, in fact, directly linked to hormonal changes. Fatigue that coffee can't fix, weight gain concentrated around the middle, sleep that fragments at 3am every night, brain fog that makes you question your own competence. These are not signs of ageing badly. They're signs of a hormonal transition that responds well to proper clinical attention.

Mood changes catch many women off guard. Anxiety that arrives without obvious cause, irritability that feels disproportionate, low mood that doesn't shift. A patient presenting with new-onset anxiety in her late 40s, no prior psychiatric history, and concurrent sleep disruption may well be experiencing perimenopause, but both possibilities need proper assessment. Getting the diagnosis right matters because the treatment path is different.

Joint stiffness, skin changes, thinning hair and reduced libido all belong on the list too. Around 80% of women will experience noticeable symptoms during the menopause transition, according to the British Menopause Society. Not all of those symptoms are obvious, and not all of them will be volunteered in a rushed appointment.

We take the time to ask about all of it. A full-picture assessment is the starting point, not a luxury.

How does a menopause consultation work?

Every consultation begins with a detailed medical history and symptom review. Your GP will ask about the pattern, severity and duration of your symptoms, your personal and family medical history, current medications and how your daily life is affected. This is not a screening questionnaire. It's a clinical conversation.

From there, your GP can assess your situation and explain the available options. These may include lifestyle and nutritional changes, sleep and stress management strategies, non-hormonal treatments, or HRT where clinically appropriate. NICE guideline NG23 is clear that management should be individualised, with a frank discussion of both benefits and risks. Costa Health's doctors follow this guidance closely.

Blood tests are not always necessary. For women over 45 with characteristic symptoms, NICE states that diagnosis can be made on clinical grounds alone. Your GP will explain whether any investigations are warranted in your case, and if blood work is needed, they'll arrange it through a local laboratory on the Costa del Sol.

Is HRT the right choice for everyone?

No. HRT is one option among several, and it isn't suitable for every woman. NICE NG23 supports its use where the benefits outweigh the risks, but that calculation depends on individual factors: your symptom severity, medical history, family history of breast cancer, cardiovascular risk and personal preferences. Blanket statements about HRT being a wonder drug, or conversely something to fear, are both unhelpful.

The GPs can assess whether HRT is appropriate for you. They can also discuss non-hormonal alternatives, including cognitive behavioural therapy for vasomotor symptoms, specific dietary adjustments, exercise programmes and, where indicated, medications such as SSRIs for mood-related symptoms. Some women do best with a combination approach

There are also women for whom HRT is clearly not appropriate, such as those with a history of certain hormone-sensitive cancers or uncontrolled blood pressure. In these cases, your GP will discuss alternative management and, if needed, refer to a specialist menopause clinic. Knowing what's off the table is just as important as knowing what's on it.

One-off consultation or membership programme?

A single consultation gives you a thorough assessment, clear guidance and a starting point. For some women, that's enough. They want to understand what's happening, hear their options and take it from there. Costa Health's GPs provide that without pressure to sign up for anything further.

Membership programmes run for three to six months and are designed for women who want continuous clinical oversight during their transition. Monthly reviews let your GP track how you're responding, adjust treatment if needed and stay ahead of new symptoms as they emerge. Members also receive recurring prescriptions, personalised updates between appointments and direct access to their doctor when questions come up.

Menopause is not a single event. It unfolds over years, and symptoms shift. Structured monitoring catches changes early and keeps your care plan current rather than reactive. Your GP can advise which approach suits your situation.

Who can access this service?

Costa Health's women's health consultations are available to anyone in Spain via telehealth. You don't need Spanish health insurance, EHIC coverage or residency. Appointments are conducted in English or Spanish depending on your preference and your chosen GP.

Dr Delgado also offers in-person appointments from Estepona, providing a face-to-face option for women on the western Costa del Sol. Dr Lake consults from various locations in Marbella and Benalmadena as well as online. Both doctors hold full UK medical qualifications and maintain their GMC registration.

If your GP determines that a video consultation isn't sufficient, or that you need investigations or specialist referral that can't be managed remotely, they'll coordinate the next step.

When should you seek urgent care instead?

Postmenopausal bleeding is a red-flag symptom. Any vaginal bleeding that occurs after 12 consecutive months without a period must be investigated promptly. Please comsult with an inperson Doctor with urgency.

Similarly, sudden severe headaches, unexplained chest pain, visual disturbances, or significant leg swelling require immediate medical attention. Call 112, Spain's emergency number, or go directly to your nearest urgencias.

What is perimenopause, and how is it different from menopause?
Perimenopause is the transitional phase before menopause, typically beginning in a woman's 40s but sometimes earlier. During this time, oestrogen levels fluctuate unpredictably, causing symptoms like irregular periods, sleep disruption, mood changes and brain fog. Menopause itself is defined as the point when a woman has not had a period for 12 consecutive months. Many of the most disruptive symptoms actually occur during perimenopause, not after menopause has been confirmed. Costa Health's GPs can assess and support women at any stage of this transition.
Can I get HRT through an online consultation?
You can discuss HRT as part of a full clinical assessment. NICE guideline NG23 recommends that HRT should be considered after an individualised discussion of risks and benefits, taking into account symptom severity, personal medical history and preferences. Not every woman will be suitable for HRT, and not every woman will want it. Your GP will explain the options clearly, including non-hormonal approaches, lifestyle changes and when specialist referral may be more appropriate.
Do I need blood tests to confirm menopause?
For most women over 45 with typical symptoms, blood tests are not required to diagnose menopause. NICE guidelines state that perimenopause and menopause can be diagnosed based on symptoms and clinical history alone in this age group. For women under 45, or where the clinical picture is unclear, hormone blood tests (including FSH) may be helpful. Your GP will advise whether testing is appropriate for your situation.
Is this service available to expats and non-residents?
Yes. You do not need Spanish health insurance or residency to use Costa Health's online GP service. Consultations are conducted in English or Spanish, so there is no language barrier. The service is available to expats, seasonal residents, holidaymakers and anyone living on the Costa del Sol.
Can menopause affect mental health?
Yes, significantly. Fluctuating oestrogen levels directly affect serotonin and other neurotransmitters, which can cause anxiety, low mood, irritability and difficulty concentrating. The Lancet menopause series (2024) found that more than 75% of women experience symptoms during the menopause transition, including mood changes, anxiety and sleep disturbance. These symptoms are often misdiagnosed as depression or anxiety disorders. Costa Health's GPs understand the hormonal basis of these changes and can help distinguish between menopause-related mood symptoms and other conditions that may need separate treatment.
What is the difference between a one-off consultation and the membership programme?

Postmenopausal bleeding, meaning any vaginal bleeding that occurs after 12 months without a period, should be investigated urgently and not through routine telehealth. This is a red-flag symptom that requires prompt assessment to rule out serious conditions including endometrial cancer. Similarly, sudden severe headaches, chest pain, unexplained leg swelling or any symptom that feels like an emergency should prompt immediate medical attention via 112 (Spain's emergency number) or your nearest hospital.
